Support at Home Care Manager (Case Manager)

About the role

Healthcare Australia is growing its client base and is looking for a proactive, people-focused Case Manager to support Home Care clients across the South-Eastern or Northern suburbs of Melbourne. As the primary point of contact, you will deliver personalised services and build strong relationships with clients and their support networks, guiding them through their Home Care journey. The role involves both office work and visiting clients in their homes.

Key responsibilities

• Meet clients in their homes and conduct assessments of their needs
• Develop care plans and coordinate services tailored to individual needs
• Work closely with internal teams to ensure high-quality service delivery
• Maintain accurate documentation and comply with compliance requirements

Required profile

• Experience in Home Care, aged care, community services, healthcare, care coordination or case management
• Knowledge of Home Care or Support at Home services is highly regarded; broader healthcare and community services backgrounds are also considered
• Organised, proactive and confident managing multiple priorities
• Excellent communication skills and a customer-first mindset
