Senior Personal Care Worker – Rural North West

About the role

A not-for-profit aged care provider is looking for an experienced Senior Personal Care Worker to join its Community Health and Care team in the Rural North West, based in Gisborne, Victoria. Working closely with the Senior Workforce Coordinator, the role combines hands-on community care with mentoring and support for Personal Care Workers, helping older people stay independent in their own homes.

Key responsibilities

• Mentor, coach and support Personal Care Workers, including the induction and development of new team members
• Conduct onsite client visits and Home Safety Assessments, identifying changes in client needs, wellbeing and potential risks
• Work with Care Partners, Scheduling and Rostering teams and Client Advisors to support high-quality service delivery
• Provide direct personal care and operational support when required, including leave coverage
• Support workforce coordination, rostering, recruitment, training and continuous improvement activities

Required profile

• Certificate III in Individual Support (Ageing) and Certificate IV in Aged Care
• Current First Aid and CPR certificates
• Strong experience in aged care or community care, with the ability to mentor, coach and support other Personal Care Workers
• Understanding of aged care quality standards, workplace health and safety and safe care delivery in clients' homes
• Ability to identify changes in client needs and risks and escalate concerns appropriately
• NDIS Worker Screening Clearance required for eligibility

• Strong organisational skills, working independently and maintaining accurate records
