Rehabilitation Consultant (Psych/Social Worker/Rehab Counsellor)

About the role

Konekt, part of APM Group, is looking for a Rehabilitation Consultant (Psychologist, Social Worker or Rehabilitation Counsellor) in Melbourne. You will support people recovering from injury or illness to rebuild confidence, independence and hope, helping them return to work and to wellness.

Key responsibilities

• Assess clients' rehabilitation needs after injury or illness through physical, ergonomic, psychosocial or functional capacity evaluations, depending on your clinical discipline
• Advise on workplace modification and the return-to-work process
• Help clients set meaningful recovery or employment goals, then co-design and deliver tailored rehabilitation plans
• Monitor progress and adjust plans to support sustainable outcomes
• Manage cases end-to-end, coordinating with employers, healthcare providers, treating practitioners and insurers

Required profile

• Tertiary qualification in Psychology, Rehabilitation Counselling or Social Work
• Registration with AHPRA (provisional acceptable), ASORC (full membership) or AASW
• National criminal check and a Working with Children check before starting
• Full Australian working rights and a driver's licence
• Willingness to work in workplaces, client homes and remote sites
• Ability to coordinate across stakeholders and a collaborative, problem-solving approach
