Registered Nurse

About the role

We are looking for a Registered or Enrolled Nurse to join our Rockhampton Medical Centre, supporting GPs and patients with chronic condition management and preventative healthcare. You will work within a collaborative, supportive team focused on patient outcomes.

Key responsibilities

• Prepare Care Plans and Team Care Arrangements.
• Support patients in managing chronic conditions and improving health behaviours.
• Conduct health assessments and promote preventative care.
• Collaborate with GPs and multidisciplinary teams.
• Manage administrative tasks related to chronic condition management appointments.

Required profile

• AHPRA‑registered Enrolled or Registered Nurse.
• Prior experience in Chronic Condition Management preferred.
• Strong communication and organisational skills.
• Friendly, proactive, and patient‑focused approach.
