Registered Nurse – Home Care

About the role

BaptistCare Home Care is seeking a compassionate Registered Nurse to deliver high‑quality care to clients living at home across Karuah, Port Macquarie and surrounding areas. You will work under the Senior Nurse Consultant, ensuring care meets aged‑care quality standards while promoting independence and safety.

Key responsibilities

• Visit clients at home to perform clinical assessments, provide nursing care and monitor ongoing health needs.
• Develop, implement and regularly review individualized care plans.
• Build supportive relationships with clients, families and carers.
• Collaborate with multidisciplinary teams, GPs and allied health professionals to coordinate care.
• Promptly act on referrals and clinical recommendations.

Required profile

• Current AHPRA registration as a Registered Nurse.
• Minimum 12 months clinical experience in acute or aged‑care settings.
• Unrestricted Australian driver’s licence with third‑party damage insurance.
• Strong written and verbal communication skills.
• Ability to work independently and as part of a team.
