Registered Nurse - Community Home & Community Services

About the role

Anglicare Southern Queensland's Cleveland Home & Community Services is looking for a full-time Registered Nurse. The nurse provides comprehensive nursing assessments, case management of clients and applies clinical knowledge to implement planned care. Clients include those funded through Support at Home, End of Life, Commonwealth Home Support, Metro South Palliative Care and Metro South Tuberculosis. The nurse works with colleagues and external community agencies to meet client needs.

Key responsibilities

• Provide nursing assessments and case management for clients under various funding structures
• Implement planned care using clinical knowledge
• Work with professional colleagues and external agencies to meet client needs
• Deliver quality nursing care within a risk management framework and professional standards (infection control, codes and competency, standards and legislation)
• Work independently with good time management and problem-solving skills
• Build strong relationships within a multidisciplinary team and take part in quality improvement activities

Required profile

• Degree in nursing and current AHPRA registration with evidence of recent practice
• Valid aged care worker screening check: current National Police Check or current NDIS Worker Screening Clearance (NDIS clearance is mandatory for services to NDIS clients), plus statutory declaration if born outside Australia after age 16
• Current driver's licence and own reliable vehicle
• Current CPR certification
• Ability to work a 24/7 rotating roster including weekends and public holidays, and to be on-call
• Active involvement in quality activities for service improvement in a clinical setting
