Part‑Time Chronic Disease Management Nurse

About the role

ForHealth Medical Centres is seeking a part‑time Chronic Disease Management (CDM) Nurse to join its Bondi Medical Centre. You will work closely with general practitioners, patients and the wider health team to deliver preventative care and support the management of long‑term conditions.

Key responsibilities

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Conduct health assessments and identify patients eligible for CDM support.
• Educate and coach patients on disease self‑management and sustainable lifestyle changes.
• Coordinate referrals and ongoing care with multidisciplinary providers.
• Partner with GPs to promote preventative health initiatives across the practice.
• Increase patient engagement in CDM programs and ensure care plans are regularly reviewed.
• Complete all administrative tasks related to CDM appointments accurately and efficiently.

Required profile

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Demonstrated experience in chronic disease management within a general practice setting.
• Experience conducting health assessments (highly regarded).
• Strong organisational and time‑management abilities.
• Excellent communication and patient‑focused interpersonal skills.
• Passion for preventative healthcare and improving patient outcomes.
