OOHC-HPP Case Manager – Social Worker, OT or Psychologist

About the role

The Out of Home Care Health Pathways Program (OOHC‑HPP) seeks a qualified professional to coordinate health assessments and interventions for children and young people entering or exiting statutory out‑of‑home care. This full‑time contract role supports vulnerable youth, improves health outcomes and enhances placement stability.

Key responsibilities

• Conduct comprehensive psychosocial and developmental assessments for children and young people in OOHC.
• Develop and implement individualized health management plans.
• Collaborate with multidisciplinary teams, carers and placement providers to ensure timely interventions.
• Monitor progress, adjust care plans and provide ongoing support until at least June 2028.
• Maintain accurate records and comply with NSW Health reporting requirements.

Required profile

• Relevant Australian‑recognised tertiary qualification (Social Work, Occupational Therapy or Psychology) with registration or eligibility for professional peak body.
• Current Australian working rights and a valid driver’s licence.
• Current NSW Working With Children Check (WWCC).
• Ability to work flexibly, including hybrid arrangements and varied hours as needed.

Required skills

• Ability to conduct comprehensive psychosocial assessments of children and young people.
• Strong written and verbal communication for multidisciplinary collaboration.
