Occupational Therapist – Home Care

About the role

Mercy Health is hiring an Occupational Therapist for its Home Care team in Hughesdale. The therapist delivers person-centred, evidence-based services that help clients maintain or improve their independence, functional capacity and quality of life. The position is permanent and full-time, with 76 hours per fortnight and a salary of $110k to $120k.

Key responsibilities

• Conduct occupational therapy assessments in clients' homes, covering functional capacity, mobility, falls risk and home safety.
• Develop, implement and review goal-focused intervention plans.
• Recommend assistive technology, equipment and home modifications.
• Deliver reablement and restorative care interventions and support clients to stay independent in daily activities.
• Work with care partners, nurses, allied health professionals, families and carers.
• Prepare clinical documentation, reports and funding recommendations.
• Contribute to quality improvement, risk management and compliance, and take part in professional development.

Required profile

• Bachelor or Master qualification in Occupational Therapy.
• Current AHPRA registration as an Occupational Therapist.
• Current driver's licence.
• Desirable: experience in aged care, community health, disability, NDIS, rehabilitation or home care, and knowledge of assistive technology, home modifications and falls prevention.
• Desirable: experience in multidisciplinary teams.
• Evidence of, or progress toward, a police record check and a Working with Children Check.
