Nurse Practitioner - Residential Aged Care

About the role

Hunter New England Local Health District is recruiting a Nurse Practitioner for its Residential Aged Care-In Reach (RAC-I) program, based at Manning Base Hospital. The practitioner works with a multidisciplinary team to provide clinical expertise to residential aged care facilities across Newcastle, Lake Macquarie, Port Stephens, Taree and the Upper and Lower Hunter, with potential for expansion across the district.

The role provides timely, specialised acute care to residents in their own aged care homes, using a mix of face-to-face and virtual consultations, so that residents receive the right care in the right place at the right time, improving health outcomes, quality of life and reducing avoidable hospital transfers.

Required profile

• Registration as a Registered Nurse with APHRA
• Nurse Practitioner qualification, or working towards a Master of Nursing (Nurse Practitioner), or a qualification or master's level deemed equivalent
• Current driver's licence
• Australian citizenship, Australian permanent residency, or New Zealand citizenship with a current passport; temporary visa holders may only be offered employment if no suitable local candidate is found, and visa sponsorship is not available
