Lifestyle Support Worker (NDIS and Aged Care)

About the role

Livable is seeking experienced Lifestyle Support Workers for its NDIS and Aged Care services in the Clarence Valley, Coffs Harbour and surrounding areas. You will work alongside participants to support their independence, choice, wellbeing and community participation, following each person's individual support plan and working with families, carers and the wider team. The position is listed as permanent with contracted hours, though the position description also refers to part-time/casual arrangements.

Key responsibilities

• Provide personal care and help with daily living activities such as bathing, showering, hygiene, dressing, grooming, eating and mobility
• Assist with domestic tasks including meal preparation, cleaning, laundry, vacuuming and shopping
• Help participants access and take part in the community, attend appointments, activities and social events, and provide transport support
• Monitor and prompt participants taking their medication in line with support plans and policies
• Complete accurate case notes and sign in and out of shifts using Livable's systems
• Follow support plans, risk assessments, behaviour support plans and workplace procedures, and maintain professional boundaries and confidentiality

Required profile

• Certificate III or higher in Individual Support, Aged Care, Disability, Community Services or another relevant qualification
• Demonstrated experience in disability support and/or aged care
• Current NDIS Worker Screening Check, Working with Children Check, First Aid certificate and CPR certification
• Current Australian driver's licence and access to a reliable, insured vehicle
• Current Australian working rights

Required skills

• Ability to complete accurate case notes and use digital systems for rostering, sign-in/out and documentation
• Confidence working independently in participants' homes and in the community
