Homecare Nurse Manager

About the role

Danone Nutricia is seeking a Homecare Nurse Manager to lead its Homecare strategy within the Healthcare Nutrition division. This permanent, full‑time, field‑based position blends clinical expertise in enteral feeding with commercial acumen to drive patient growth, retention, and service excellence across hospital, community, aged‑care and NDIS settings.

Key responsibilities

• Execute the Homecare strategy across the assigned territory, creating and delivering targeted key‑account plans.
• Identify commercial opportunities and risks using data insights, referral patterns and market knowledge.
• Build and influence strategic relationships with dietitians, nurses, discharge teams and case managers.
• Facilitate hospital‑to‑community patient transitions and secure appropriate enteral nutrition pathways.
• Generate referrals, optimise therapy and retain patients through high‑quality clinical engagement.
• Deliver clinical education and training to healthcare professionals on enteral nutrition and Nutricia products.
• Monitor activity and performance using reporting tools to meet clinical and commercial KPIs.
• Provide hands‑on patient care, including tube changes, stoma care and troubleshooting.
• Contribute to continuous improvement of the Nutricia Nursing Service.
