Health Care Worker - Community Support

About the role

Portland District Health is looking for a Health Care Worker to join its Community Aged Care Services team in Portland. This is a fixed-term part-time position of 12 to 76 hours per fortnight, available now until 30 June 2027, with a closing date of 30 November 2026.

Key responsibilities

• Assist consumers with light, general housekeeping such as sweeping, vacuuming, mopping, dusting, wiping, bed making, laundry and limited shopping.
• Provide in-home respite or take part in social activities such as shopping, outings and walks, including one-on-one and group activities, to help people stay active and connected.
• Encourage consumers to maintain and enhance their abilities and strengths, and to stay independent by participating as much as possible.
• Support each person's choice to remain at home and take part in their community.
• Carry out no personal tasks that require the skills of a qualified nurse or allied health professional, and no tasks outside your own skills and competence.

Required profile

• Certificate III or Certificate IV in Aged Care from an Australian registered training organisation, or a recognised equivalent.
• Current Victorian driver's licence.
• Previous experience in a hospital, community care or residential aged care setting.
• Ability to work collaboratively in a multidisciplinary health care team and to deliver person-centred care in line with registration requirements and PDH policy and procedures.
• Understanding of the needs and sensitivities of people from diverse backgrounds and cultures, and excellent communication skills.
