Entry-Level Care Manager (Aged Care)
Care Connect
Job description
About the role
Care Connect, a not-for-profit home care specialist active since 1994, is seeking a Care Manager for its Aged Care team on a 12-month maximum-term, full-time contract. You will support a caseload of clients across Greater Sydney, working with older Australians and their families to understand their goals, assess their needs, develop personalised care plans and connect them with the services and supports they need to live independently at home.
You report to the Senior Care Manager and build trusted relationships with clients, families and stakeholders, giving ongoing advice and ensuring services stay responsive to changing needs.
Key responsibilities
- Complete client assessments and develop person-centred care plans.
- Support clients to access services that promote independence, choice and wellbeing.
- Monitor client needs and carry out ongoing care plan and budget reviews.
- Build relationships with clients, families, service providers and referral partners.
- Work with Service Coordinators so services are delivered safely and effectively.
- Maintain accurate client records and documentation.
- Advise clients so they can make informed decisions about their care.
- Identify ways to improve client outcomes and service delivery.
Required profile
- Minimum Diploma in health, social or community services, or working towards a relevant qualification; recent graduates and entry-level candidates are encouraged to apply.
- Strong communication, planning, organisational and time-management skills, and attention to detail.
- Ability to manage competing priorities and work autonomously.
- Highly regarded but not essential: experience in aged care, community services, health, disability or human services, knowledge of Home Care Packages, and care management or support coordination experience.
Required skills
- Confident use of Microsoft Office and other business systems.
- Accurate record-keeping.
What we offer
- 12-month maximum-term contract with full-time hours and hybrid flexibility.
- Competitive salary with NFP salary packaging benefits.
- 5 weeks' annual leave and ADO option.
- Ongoing learning and professional development, Employee Assistance Program, discounted health insurance and wellbeing benefits including Fitness Passport.
Applicants must provide a current police check (organised and funded by the employer), a Working with Children Check, a current driver's licence, evidence of the right to work in Australia, and a reliable vehicle with comprehensive car insurance. Applications are reviewed on an ongoing basis.
