Enrolled Nurse - Transition Care Service

About the role

Amplar Health, part of the Medibank Group, runs a Transition Care Service that supports patients as they recover, build confidence and prepare for the next step in their care. It is recruiting Enrolled Nurses for its home hospital service, based at either the Adelaide CBD or Walkerville location, in a supportive, multidisciplinary environment with experienced senior staff and a Nurse Educator.

The work follows a seven-day rotating roster, including morning, afternoon and night shifts. Permanent positions are available from 0.6 FTE through to full-time.

Key responsibilities

• Deliver compassionate, person-centred nursing care
• Monitor patient progress and recognise and report clinical changes
• Contribute to assessments and individualised care planning
• Administer medications within your scope of practice
• Support wound care and activities of daily living
• Work alongside registered nurses and the wider multidisciplinary team
• Maintain accurate clinical documentation

Required profile

• 1+ years' experience as an Enrolled Nurse
• Current unrestricted AHPRA registration
• Experience in wound care
• Current CPR certification
• SA Working with Children Check and National Police Clearance, or willingness and eligibility to obtain them
• Ability to provide evidence of required vaccinations if successful
