Registered Nurse - Transition Care Service

About the role

Amplar Health, part of the Medibank Group, is recruiting Registered Nurses for its Transition Care Service. The service supports patients as they recover, build confidence and prepare for their next stage of care, combining hands-on patient care with multidisciplinary teamwork and clinical variety. The role suits experienced RNs who want to move beyond acute care, and permanent full-time and part-time positions are available from 0.6 FTE, based in Adelaide CBD or Walkerville.

Key responsibilities

• Deliver high-quality care across a broad mix of clinical presentations
• Support patients through recovery and transition
• Work closely with a multidisciplinary healthcare team
• Assess, escalate and respond to changes in clinical condition
• Use technology to support contemporary care
• Mentor and support colleagues and continue developing your own clinical capability

Required profile

• 1+ years' nursing experience
• Unrestricted AHPRA registration
• Current First Aid and CPR
• SA Working with Children Check and National Police Clearance, or eligibility to obtain them
• Able to meet vaccination evidence requirements

The roster is seven days a week, rotating across mornings, afternoons and nights.
