Case Coordinator - Virtual SA Community Care

About the role

Amplar Health, the unified health services brand of Medibank, is growing its hospital-in-the-home and clinical home care delivery. It is offering a full-time virtual Case Coordinator position within the SA Community Care (SACC) program, based in Adelaide but open to interstate applicants. You will connect patients, families and providers so that care is coordinated, timely and patient-centred.

Key responsibilities

• Conduct initial phone assessments, care planning and goal setting
• Coordinate in-home services for clients receiving care at home
• Manage escalations and support frontline community nurses
• Manage consumer care across the journey: on-boarding, triaging referrals, initial care planning, care plan review and discharge
• Provide telephone support, education and advice
• Plan discharges, including referrals to long-term services
• Monitor patient progress, identify risks and escalate concerns appropriately
• Collaborate with multidisciplinary teams to ensure continuity of care
• Maintain accurate clinical documentation and reporting in line with standards

Required profile

• Registered Nurse with current unrestricted AHPRA registration
• Experience in an acute setting (community or hospital)
• Strong understanding of care coordination, case management or complex care (highly desirable)
• Digital proficiency and ability to work virtually
• Excellent communication and relationship-building skills
• Confident working independently while collaborating in a multidisciplinary team
• Highly organised, with strong clinical judgment and problem-solving skills

What we offer

• True flexibility: rostered across 7 days, working between 7am and 9pm in 4-hour shifts (7am-3pm, 10am-6pm, 12pm-8pm, 1pm-9pm)
