Community Clinical Nurse – Rural Aboriginal Health

About the role

The Clinical Nurse will work autonomously within the Woorabinda community, delivering direct nursing care to Aboriginal residents in a rural and remote setting. You will join a multidisciplinary Community Health Team to design, implement and evaluate health programs that address chronic disease, women and child health, public health and home support services.

Key responsibilities

• Provide comprehensive nursing assessments and interventions for patients with chronic conditions, especially diabetes.
• Deliver culturally safe, holistic care that respects the impacts of colonisation.
• Collaborate with community members, partner agencies and health professionals to improve health outcomes.
• Participate in community health initiatives, immunisation programs and health education sessions.
• Maintain accurate clinical documentation and adhere to organisational policies.

Required profile

• Current registration as a Registered Nurse with AHPRA and a valid practising certificate.
• Willingness to obtain or hold qualifications related to diabetes education (preferred).
• Experience in community, primary health care or rural/remote nursing is advantageous.
• Immunisation program nurse qualification is a plus.
