Clinical Nurse Discharge Planner (Patient Access and Clinical Effectiveness)

About the role

Caboolture Hospital is looking for a Clinical Nurse Discharge Planner to join its Patient Access and Clinical Effectiveness (PACE) team. You will coordinate complex patient discharges and ensure safe, timely transitions across healthcare services, working with patients, families, multidisciplinary teams and external providers. The position is part-time and permanent.

Key responsibilities

• Coordinate complex discharge planning to support safe transitions of care across acute, subacute, community and primary services.
• Apply advanced clinical assessment, planning, implementation and evaluation skills to deliver patient-centred care.
• Advise on community services, ongoing care pathways and referral processes.
• Collaborate with multidisciplinary teams, patients and families to integrate care.
• Lead quality improvement initiatives, provide clinical support and education and contribute to evidence-based practice within the PACE service.
