Care Partner - Home Care

About the role

Respect, Regional Australia's largest aged care provider, is looking for a Care Partner for its Independence at Home team in Launceston. This full-time, Monday to Friday role supports older Australians to live safely and independently in their own homes, building meaningful connections and promoting dignity and wellbeing.

Key responsibilities

• Deliver safe, person-centred, enablement-focused case management that promotes independence, wellbeing and community connection.
• Conduct assessments, develop individualised care plans and complete regular reviews in line with participant goals and Support at Home requirements.
• Monitor wellbeing, identify and escalate risks, and coordinate referrals with clinical teams, allied health and community services.
• Coordinate care transitions, including restorative, palliative and end-of-life pathways.
• Oversee service coordination, participant budgets and workforce scheduling, keeping accurate records and meeting legislative and organisational compliance, risk management and complaints handling.

Required profile

• Minimum 3 years' experience in case management, aged care, home care or a related field, with knowledge of the Aged Care Quality Standards, Code of Conduct and accreditation requirements.
• Strong communication and interpersonal skills, able to build stakeholder relationships and advocate for participant dignity and choice.
• Ability to work independently, exercise sound judgement and solve problems in a fast-paced environment.
• Experience in budgeting, funds management and service coordination, including risk management, hazard identification and incident reporting.
• Registered Nurse with current AHPRA registration is desirable but not essential.
