Care Coordinator - Registered Nurse (Part Time)

About the role

Amplar Health, part of the Medibank Group, is recruiting a Care Coordinator Registered Nurse for its Virtual Care Centre in Eastwood, South Australia. The role is part time (0.8 FTE) and involves establishing, monitoring and coordinating acute patients' treatment plans using virtual technology, in partnership with Medical Officers and a home visiting provider team.

Key responsibilities

• Establish, monitor and coordinate acute patients' treatment plans using virtual technology
• Work with Medical Officers and the home visiting provider team to deliver patient-centred virtual care
• Manage patient care using clinical, people and communication skills to enhance quality of life

Required profile

• 3 years of acute hospital experience as a Registered Nurse
• Current unrestricted AHPRA registration
• Ability to think on your feet and problem solve
• Willingness to work a 7-day rotating roster between 7am and 10pm
• South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain them)
• Willingness to provide proof of up-to-date vaccinations if requested

• Proficient computer skills for fast and accurate data entry into patient electronic medical records
