Social Worker - Transition Care Service

About the role

Amplar Health, part of the Medibank Group, is recruiting a full-time Social Worker to deliver comprehensive care for patients with varying complexity in a sub-acute setting, based centrally in Adelaide's CBD. The role uses innovative technology and contributes to exceptional health outcomes for sub-acute patients.

Key responsibilities

• Collaborate with a multidisciplinary team to develop and implement integrated service models and clinical protocols.
• Facilitate group sessions and contribute to health promotion activities as required.
• Provide assessment and triage, timely referrals, accurate information, coordinated care and prompt follow-up.
• Facilitate discharge planning.
• Advocate for patients' needs and coordinate access to services.
• Take part in clinical review meetings and continuous professional development.

Required profile

• Degree in Social Work with membership of the Australian Association of Social Workers (AASW).
• Demonstrated experience as a Social Worker in a sub-acute or similar healthcare setting.
• Strong communication, advocacy and problem-solving skills, with excellent interpersonal skills and the ability to adapt to changing service needs.
• South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain them).
• Willingness to provide proof of up-to-date vaccinations if requested.

Required skills

• Proficiency in Microsoft Office and other relevant documentation systems.
