Care Coordinator

About the role

Geneva Healthcare's Homecare Group provides care and support in people's own homes across New Zealand, including for people with disabilities, those recovering from illness or accidents and the elderly. As Care Coordinator you will be the first point of contact for support workers on the field and for clients, managing high call volumes inbound and outbound to meet KPIs and deliver strong first-call resolution. The position is permanent and full-time, Monday to Friday, 8:30am to 5:30pm, in the CBD office in Tāmaki Makaurau, Auckland.

Key responsibilities

• Manage all clients in your allocated business units or regional area
• Place the most suitably trained support worker with each client
• Provide proactive, planned responsiveness to clients and support workers
• Document all written and verbal communications to manage incidents
• Assess new complex clients and ensure the Complex Assessment team follows its plans
• Build and manage client and support-worker relationships, keeping communication ongoing
• Keep client and support-worker records up to date
• Manage all aspects of client services
• Ensure support workers are signed off as clinically competent, including scheduling sign-offs
• Manage planned support-worker leave in line with company policy so clients receive timely updates and continued care

Required profile

• Proven experience in a call centre environment
• Experience in a health administration or coordinator role is preferred
• Scheduling or rostering experience is advantageous
• Excellent communication skills and a genuine interest in helping others
• Able to multitask and resilient under pressure
• NZ citizen or resident, or holds a valid NZ work visa
