Assessment Coordinator

About the role

Mercy Health is looking for an Assessment Coordinator in the HCS Barwon Region area. This is a casual position responsible for conducting client assessments, accessing respite services, supporting service development and setting up client rosters. The role sits within a multi-disciplinary team serving consumers and carers.

Key responsibilities

• Conduct client assessments and access respite services
• Support the growth of the program, building on the current CHSP program and growing private client case loads
• Review procedures, administration and reporting requirements together with program stakeholders
• Provide highly responsive and flexible service to consumers and carers

Required profile

• Relevant community care experience in intake, assessment and care planning
• Excellent knowledge of CHSP, NDIS, VHC, DVA and HCPs
• Understanding of the needs of people from special needs groups
• Ability to nurture friendly, respectful relationships and work collaboratively
• Resilient, compassionate and committed to client-centred care
• Evidence of, or progress toward, an NDIS workers screening check

Required skills

• Time management, organisational and communication skills, with the ability to adapt and problem solve
• Experience with the Microsoft Office suite
