Team Leader – Home Care (Queensland) – Permanent

About the role

MiCare is seeking a Team Leader for Home Care in Queensland to lead the delivery of high‑quality in‑home services for Support at Home and Veterans Home Care clients. This permanent, full‑time role combines operational leadership with direct case management to ensure culturally appropriate, person‑centred care.

Key responsibilities

• Ensure delivery of high‑quality, person‑centred care aligned with Support at Home and VHC program requirements.
• Maintain accurate, up‑to‑date client documentation, including care plans, reviews and progress notes.
• Lead, coach and support SAH Care Partners and the Veterans Home Care Coordinator.
• Oversee intake processes, onboarding, assessments and care‑plan development.
• Ensure accurate data entry and client profile creation in the client management system.
• Monitor quarterly budgets to keep services within allocated limits.

Required profile

• Tertiary qualification in Social Work, Nursing, Aged Care, Disability, Allied Health, Business Management or a related field.
• Current professional registration where required (e.g., AHPRA).
• Management or leadership qualification or equivalent workplace experience.
• Demonstrated leadership experience in aged care, community services or related human services.
• Strong knowledge of the Aged Care Quality Standards and health‑and‑safety requirements.
• Familiarity with Support at Home and Veterans Home Care program rules and guidelines.
