Support Coordinator

About the role

Quality Care Network is seeking experienced and emerging Support Coordinators to help participants navigate the NDIS with confidence. You will work closely with participants, families and service providers to implement NDIS plans, build capacity, coordinate supports and promote choice and control.

Key responsibilities

• Implement NDIS plans
• Coordinate formal and informal supports
• Build participant capacity and independence
• Support participants through plan reviews and changes of circumstances
• Liaise with allied health professionals, providers and government agencies
• Maintain accurate documentation and participant records

Required profile

• Experience in Support Coordination, Case Management, Social Work, Psychology, Occupational Therapy, Nursing or Community Services
• Excellent organisational and problem-solving skills
• Confident managing complex stakeholder relationships
• Committed to recovery-oriented and strengths-based practice
• Able to work independently in a supportive team environment
• Desirable: degree or qualification in Social Work, Psychology, Community Services, Occupational Therapy, Nursing or a related discipline; current NDIS Worker Screening Check; current driver's licence
