Registered Nurse - Transition Care

About the role

Amplar Health Home Hospital, part of the Medibank Group, is expanding its Transition Care Service and is recruiting Registered Nurses to join the team. The role sits outside the hospital, in a sub-acute setting, supporting patients as they recover or improve their health outcomes. Positions are permanent, full-time or part-time (minimum 0.6 FTE), based in Adelaide's CBD or Walkerville.

Key responsibilities

• Deliver high-quality patient care across a broad range of clinical presentations
• Coordinate and deliver patient-centred care in a multidisciplinary environment
• Work in a professionally simulated, rewarding environment using innovative technology
• Maintain accurate patient records
• Respond confidently to clinical changes and emergency situations
• Mentor junior staff and contribute to continuous improvement
• Take part in ongoing professional development and training

Required profile

• At least 1 year of experience as a Registered Nurse; nurses with acute care experience are encouraged to apply
• Current unrestricted AHPRA registration
• Current First Aid and CPR certificates
• South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain them)
• Willingness to provide proof of up-to-date vaccinations if requested
