Registered Nurse - Support At Home

About the role

As a member of the Calvary Home Care team, you will deliver quality nursing services to Support at Home clients in the Adelaide community. You will work alongside multidisciplinary professionals to assess, plan and evaluate client needs, promoting positive health outcomes. The role involves providing clinical support, collaborating with support workers and care leads to ensure excellent care delivery.

Key responsibilities

• Conduct clinical assessments of clients in their homes.
• Support care delivery alongside support workers.
• Develop and update individualized care plans.
• Provide clinical oversight for complex cases.

Required profile

• AHPRA registration and a current First Aid Certificate.
• Graduate nurses are welcome to apply.
• Strong communication, interpersonal and organisational abilities.
• Initiative and a commitment to continuous improvement of client care.
• Ability to prioritise tasks and meet deadlines.
• Understanding of community nursing, Support at Home, Department of Veteran Affairs and Commonwealth Home Support Program is advantageous.
