Registered Nurse - Community Care

About the role

Bayside Health, a public health service covering metropolitan Melbourne and surrounding areas, is recruiting a Registered Nurse (Grade 3 HITH/PAC) for its Community Care team at Peninsula University Hospital, Frankston. This is a limited-term part-time position available until June 2027. The nurse coordinates care for patients and their families across the organisation, working in community settings, residential aged care facilities and clients' homes, and may be allocated in-charge shifts when needed.

Key responsibilities

• Provide outreach services in community settings, residential aged care facilities and clients' homes
• Deliver safe, evidence-based, person-centred care within the scope of practice, aiming for optimal health outcomes or a dignified death
• Facilitate collaborative, integrated care planning across acute and community-based services
• Work flexibly with a 7-day rotating roster, with AM shifts 0800-1630 and PM shifts 1230-2100
• Carry out all aspects of the role as outlined by the Australian Health Practitioner Regulation Agency, with excellent assessment skills

Required profile

• AHPRA-registered nurse
• Demonstrated clinical knowledge and skills aligned with the role
• Experience working with at-risk and vulnerable people with complex needs
• Understanding of client health literacy and its impact on care management
