Registered Nurse Care Coordinator

About the role

Teachers Health is looking for an experienced Registered Nurse Care Coordinator to join its Healthcare Services team. This is a specialist telephone-based care coordination role that supports members in navigating their health needs, building ongoing relationships, developing personalised care plans, providing health coaching and connecting members with the right services and support.

Key responsibilities

• Identify and engage eligible members through referrals, enquiries and outreach programs
• Support the assessment, development and monitoring of personalised care plans
• Coordinate care with members, healthcare providers, service providers and broader care teams
• Provide health education, guidance and support within your clinical scope of practice
• Maintain accurate clinical records and contribute to quality care outcomes
• Take part in clinical audits, professional development and continuous improvement initiatives
• Contribute to a positive, safe and member-focused team culture

Required profile

• Current unrestricted AHPRA registration as a Registered Nurse
• 5+ years' full-time equivalent experience as a Registered Nurse
• Knowledge of primary and community care environments
• Experience in telehealth, private health insurance, hospital discharge planning or customer service is highly regarded

Required skills

• Strong verbal and written communication skills
• Health coaching knowledge and capability
• Computer literacy and competency across the Microsoft Office suite
