Physiotherapist - Rehab at Home (Grade 2)

About the role

St Vincent's Health Australia is recruiting an experienced Grade 2 Physiotherapist for its Rehab at Home team in Werribee, South West Melbourne. The program delivers hospital-level rehabilitation in patients' homes, combining face-to-face care with virtual health technologies, supporting early discharge after elective orthopaedic surgery and acute private hospital admissions.

Key responsibilities

• Deliver physiotherapy assessments and evidence-based rehabilitation programs in patients' homes
• Support patients recovering from elective orthopaedic surgery and acute private hospital admissions
• Develop personalised treatment plans that maximise recovery, mobility and independence
• Use virtual care and digital health technologies to support continuity of care
• Collaborate with medical, nursing and allied health professionals
• Support early supported discharge and hospital avoidance initiatives
• Maintain accurate clinical documentation and contribute to quality improvement

Required profile

• Current AHPRA registration as a Physiotherapist
• Minimum 3 years of post-graduate physiotherapy experience (essential)
• Experience in rehabilitation, orthopaedics, subacute care or community-based services highly regarded
• Confidence working independently in community settings
• Australian driver's licence and access to a reliable motor vehicle

Required skills

• Excellent clinical reasoning and evidence-based decision-making
• Communication, organisation and time management
• Experience with digital health platforms or telehealth, or willingness to adopt new technologies
