Physiotherapist – Home Care

About the role

Mercy Health is recruiting a Physiotherapist for its Home Care service, working 9am to 5pm in Melbourne's north-west metropolitan area and Geelong. The physiotherapist provides person-centred care, assessing clients and designing personalised programmes to maximise their function, independence and quality of life.

Key responsibilities

• Independently assess clients in the home care setting and determine their individual capabilities
• Design personalised treatment programmes and strategies for common concerns such as decreased mobility, falls and pain
• Support clients, their formal and informal carers and their health goals and care plans
• Take part in client care planning, service development and quality improvement within the multidisciplinary team
• Oversee junior staff and Allied Health Assistants where required

Required profile

• Recognised tertiary qualification and current full Physiotherapy registration with AHPRA
• Demonstrated experience in home care and residential aged care
• Ability to work independently and within a team
• Commitment to ongoing professional development
• In the process of obtaining, or able to provide: proof of current influenza vaccination, a current National Police Check and a current driver's licence
