Physiotherapist Grade 2 - Transition Care Program

About the role

Bayside Health is recruiting a Grade 2 Physiotherapist for its Transition Care Program (TCP) at Golf Links Road, Frankston, Victoria. TCP is a multidisciplinary service supporting older people at the interface between hospital, home and the community. This is a fixed-term full-time position of 80 hours per fortnight until December 2027.

Key responsibilities

• Deliver physiotherapy assessment and management across a broad range of aged care clinical streams
• Help optimise patient outcomes and support safe transitions from hospital care
• Work within a collaborative multidisciplinary team to contribute to person-centred care, goal setting, rehabilitation and service improvement
• Support positive outcomes for older people in community and home-based settings across the Mornington Peninsula

Required profile

• Bachelor of Physiotherapy or Masters of Physiotherapy Practice
• Current general registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• At least 2 years of professional experience working with older adults
• Demonstrated participation in professional development
