Physical Health Nurse (Community Mental Health)

About the role

A passionate and experienced Registered Nurse is sought to improve the physical health outcomes of consumers with mental health conditions. Working across community mental health teams in Kew and East Melbourne, the nurse supports an integrated, holistic model of care, partnering with multidisciplinary teams in Hawthorn and Clarendon Community Mental Health Services to identify, monitor and manage physical health needs of people with complex and enduring mental illness. The position is full time and fixed term until January 2027.

Key responsibilities

• Conduct comprehensive physical health assessments, identifying unmet needs and contributing to holistic care planning.
• Provide physical health interventions such as wound care, metabolic monitoring, chronic disease management support and facilitation of pathology.
• Support early identification of physical health risks, including cardiometabolic concerns aligned with Equally Well principles.
• Work with consumers in community and home-based settings, encouraging engagement with primary and specialist care.

Required profile

• Currently registered with the NMBA (per EBA).
• At least 2 years' experience from an inpatient setting.
• Postgraduate diploma in psychiatric/mental health nursing or a specialist undergraduate mental health nursing qualification.
• Valid nationally coordinated criminal history check, or willingness to obtain one.
