Nurse Practitioner - New General Practice, Bassendean

About the role

Care Medical Practice is establishing a new independent general practice in Bassendean, Western Australia, and is seeking an experienced Nurse Practitioner to join the clinical team on a full-time, onsite basis. The Nurse Practitioner provides patient-centred primary healthcare within their authorised scope of practice, assessing and managing patients independently where appropriate and working with General Practitioners and other healthcare professionals. The role reports to the Director / Practice Manager and supports patient access, chronic disease management, preventative healthcare and the development of the practice.

Key responsibilities

• Conduct comprehensive patient assessments, diagnose and manage acute and chronic conditions, and develop, implement and review care plans.
• Prescribe medicines within endorsed scope, order and interpret pathology and diagnostic tests, and refer patients to GPs, specialists and allied health when appropriate.
• Provide preventative healthcare, health screening, education and lifestyle advice, and support chronic disease management.
• Maintain accurate clinical records and collaborate with GPs and nurses to coordinate care, escalating or referring when needed.
• Practice in line with NMBA standards, AHPRA registration and professional indemnity insurance, and take part in quality improvement, audits and accreditation.
• Help set up Nurse Practitioner workflows, develop chronic disease and preventative health services, and improve patient access.

Required profile

• AHPRA registration as a Registered Nurse with Nurse Practitioner endorsement.
• Demonstrated experience in advanced clinical assessment and autonomous practice; current professional indemnity insurance.
• Australian work rights.
• Desirable: previous experience in Australian general practice, chronic disease management, Medicare billing and private practice NP services, and familiarity with practice management software.
