Home Medicines Review (HMR) Pharmacist – Flexible Locations

About the role

We are looking for an accredited Home Medicines Review (HMR) Pharmacist to join our growing network on a contract basis. You will receive GP‑referred patients through the Chemist2U platform and conduct in‑person medication reviews in your preferred local area, helping to optimise patient outcomes.

Key responsibilities

• Conduct comprehensive Home Medicines Review consultations for GP‑referred patients.
• Liaise with patients to arrange and perform in‑person medication reviews.
• Prepare structured, high‑quality reports using provided templates.
• Collaborate with GPs and other healthcare professionals to improve medication safety and adherence.
• Provide practical, patient‑centred medication advice and education.

Required profile

• Current registration with AHPRA as a pharmacist.
• HMR accreditation (or eligibility to obtain it).
• Strong clinical knowledge of pharmacology and medication management.
• Experience delivering patient consultations and education.
• Excellent communication and report‑writing abilities.
• Comfortable using digital tools for telehealth and documentation.
• Ability to work collaboratively with GPs and healthcare teams.
