Home Care Coordinator (Enrolled Nurse)

About the role

Southern Cross Care is hiring a Home Care Coordinator (Enrolled Nurse) in Darwin, Northern Territory, on a permanent full-time basis of 76 hours per fortnight. Week 1 and 2 are Monday to Friday, 0800 to 1606. The coordinator is the central point of contact for clients, making sure their services run smoothly according to their needs, plans and budgets.

Key responsibilities

• Conduct client assessments, reviews and onboarding with a health-promoting focus.
• Provide nursing services within the scope of practice for Enrolled Nurses.
• Coordinate daily check-ins, service requests, equipment purchases and vendor bookings.
• Support clients from Culturally and Linguistically Diverse and Aboriginal and Torres Strait Islander backgrounds.
• Travel to clients within the region using your own registered, insured and roadworthy car.

Required profile

• Diploma of Nursing and current unrestricted AHPRA registration.
• First Aid (HLTAID011) and CPR (HLTAID009) certificates.
• Current Australian driver's licence and your own registered, insured car.
• Demonstrated experience as an Enrolled Nurse in aged care or home care.

Required skills

• Strong communication skills.
• Basic knowledge of Aged Care Standards and My Aged Care systems.
