Enrolled Nurse - Transition Care Service

About the role

Amplar Health Home Hospital is expanding its Transition Care Service (TCS) and is recruiting Enrolled Nurses to work full-time or part-time (minimum 0.6 FTE) in a sub-acute, home-based setting. The role suits nurses who want to apply their clinical knowledge in a varied, person-centred environment while learning new skills under the support of an experienced Nurse Educator.

Key responsibilities

• Monitor patients' progress and report changes to a broader multidisciplinary team
• Maintain up-to-date documentation
• Contribute to care planning and patient assessments
• Support activities of daily living and administer medications

Required profile

• 1 year or more of experience as an Enrolled Nurse
• Current unrestricted AHPRA registration
• Certification to dispense medication
• Experience in wound care management
• Current CPR certificate
• South Australian Working with Children Check and National Police Clearance (or willingness and eligibility to obtain them)
• Willingness to provide proof of up-to-date vaccinations if requested
• Nurses with agency experience in acute care are encouraged to apply
