Disability Support Worker

About the role

Care & Connects is an NDIS provider offering Supported Independent Living (SIL) homes and support for people with significant and challenging behaviours. It is looking for experienced, adaptable and resilient support workers to assist participants with daily activities in supported accommodation, such as meals, personal care, domestic tasks and community engagement. The role is hands-on and builds genuine relationships with participants and co-workers.

Key responsibilities

• Provide personalised care and support, including personal hygiene, meal preparation, medication and activities of daily living.
• Implement positive behaviour support plans and crisis intervention strategies.
• Provide emotional and practical support according to each participant's individual needs.
• Monitor participant progress and report changes or concerns as required.
• Maintain accurate records and documentation in line with NDIS and organisational requirements.

Required profile

• Minimum qualification of Certificate III (Certificate III in Individual Support).
• Experience assisting individuals with exceptional behavioural needs.
• Adaptable, resilient and able to prioritise participants' needs.
• Willingness to work the listed shifts, including short shifts of 2 to 3 hours, day shifts, afternoon shifts, rotating weekends and 24/7 rotation.

Required skills

• Medication Administration Accreditation
• NDIS Worker Screening and WWC check
• Manual handling training
• First Aid and CPR
• CPI/MAPA/Mental Health First Aid is desirable; PEG, bowel, diabetes training is an advantage
• 100 Points ID and an Australian driver's licence (at least P2)
