Clinical Nurse Consultant - Residential In Reach

About the role

The Residential In Reach (RIR) service, established by St Vincent's Hospital Melbourne in 2011, provides early intervention for older people with acute illness linked to chronic disease and ageing, working with aged care facilities, GPs and community health teams. This part-time, fixed-term position in Fitzroy aims to assess and manage residents of residential aged care facilities and the community so they do not present to the emergency department, and to advise nursing and care staff. It runs 32 hours per fortnight until June 2027.

Key responsibilities

• Provide comprehensive assessments, care planning, coordination and treatment for residents facing medical issues that could lead to an emergency department presentation.
• Give clinical advice to RACF staff, patients, carers and other health professionals, and evaluate treatment outcomes.
• Triage and screen patient referrals by clinical need and level of support, collaborating across at-home services.
• Support timely discharge of RACF residents from the ED or inpatient units and arrange follow-up to reduce future presentations.

Required profile

• Current nursing registration with AHPRA.
• Other relevant postgraduate nursing training in aged care or acute/critical care, or working towards it.
• Demonstrated advanced skills in assessing older people.
• Strong interpersonal, communication and leadership skills.
• Current Victorian driver's licence or equivalent.
