Clinical Nurse Consultant – Cardiac (Hearts at Home)

About the role

St Vincent’s Health Australia is seeking a Clinical Nurse Consultant (Cardiac) to join the innovative Hearts at Home program. This 12‑month fixed‑term role supports patients with heart failure through virtual and home‑based care, aiming to reduce hospital stays and readmissions.

Key responsibilities

• Conduct evidence‑based clinical assessments and develop individualized care plans for heart‑failure patients.
• Deliver education, coaching, and self‑management support using virtual care platforms and remote monitoring tools.
• Coordinate with hospital and primary‑care teams to ensure seamless transitions and continuity of care.
• Contribute to service development, quality improvement, and innovation within the virtual care model.

Required profile

• At least 5 years post‑registration nursing experience, including 3 years in cardiac care, chronic disease management, or critical care.
• Demonstrated experience with digital health solutions and remote patient monitoring.
• Strong commitment to compassionate, patient‑centred care and continuous improvement.
