Clinical Care Partner (Locum, Six Weeks)

About the role

Tradewind Australia is recruiting, through a partnership with a respected not-for-profit aged care organisation, an experienced Clinical Care Partner for a six-week locum assignment in Hawthorn. The role suits an experienced clinician with strong knowledge of the Support at Home (SAH) Program and a background in community aged care. It is an immediate-start opportunity.

Key responsibilities

• Conduct clinical assessments and develop individualised care plans.
• Coordinate and monitor the delivery of safe, high-quality care and services.
• Work with GPs, allied health professionals and other service providers.
• Monitor client needs and outcomes so care plans stay relevant and responsive.
• Maintain accurate documentation and comply with aged care and Support at Home requirements.
• Provide guidance and support to clients, families and carers, promoting choice, independence and wellbeing.

Required profile

• Bachelor of Nursing, Diploma of Enrolled Nursing or a relevant Allied Health qualification.
• Extensive experience in the Support at Home Program.
• Strong background in community aged care, care coordination and clinical assessment.
• Sound knowledge of aged care requirements, care planning and service delivery.
• Excellent communication, organisation and stakeholder engagement skills.
• Ability to work autonomously in a supportive team environment.
