Chronic Disease Management Nurse

About the role

We are seeking an experienced Registered Nurse to join our Royal Park Medical Centre as a Chronic Disease Management (CDM) Nurse. In this role you will act as a champion for preventative health, supporting GPs and patients to identify, manage and prevent chronic conditions, helping keep the community out of hospital.

Key responsibilities

• Prepare Care Plans and Team Care Arrangements to support disease management.
• Empower patients to take ownership of their health and coach them on behaviour change.
• Facilitate access to a multidisciplinary team for ongoing care.
• Conduct health assessments to identify early signs of chronic disease.
• Collaborate with GPs to promote preventative healthcare throughout the practice.
• Increase the number of patients receiving a first‑time care plan and ensure regular reviews.
• Manage administrative tasks related to CDM appointments.
• Participate in bonus schemes based on performance.

Required profile

• AHPRA‑registered Enrolled or Registered Nurse.
• Proven experience in chronic disease management and health assessments.
• Strong organisational and time‑management abilities.
• Excellent communication and customer‑service skills.
• Passionate, personable approach to patient care.
