Care Team Leader – Osborne Park

About the role

Silverchain is looking for a Care Team Leader for a 6-month secondment/fixed-term contract in Osborne Park, covering North-based clients. Reporting directly to the Community Manager, the role covers a variety of portfolios, clients and teams and supports and mentors direct care staff to deliver quality care to clients and carers. Travel within the community is required.

Key responsibilities

• Lead, mentor and support direct care staff to ensure best-practice service delivery
• Manage performance, onboarding, communication and timesheet approvals
• Promote a safe, positive workplace culture and ensure compliance with policies and procedures
• Allocate resources effectively and respond to staff and client queries

Required profile

• Strong background in the Aged Care industry
• Understanding of health care within a community environment
• Demonstrated experience of care management in a Support at Home program and leading a team to achieve outcomes
• Certificate III in Community Support Services is desirable, or minimum two years industry experience and willingness to obtain Cert 3
• Good communication, decision-making, time management and manual handling knowledge, including the ability to train others
• National Police Clearance under the Aged Care Act 1997 required before starting
