Care Partner

About the role

HammondCare is looking for a full‑time Care Partner to join its Home Care Team in Miranda. You will develop and maintain tailored care plans for clients, ensuring their needs and preferences are continuously assessed.

Key responsibilities

• Develop and update individual care plans in consultation with stakeholders.
• Monitor client‑related KPIs and ensure delivery of services within budget and expectations.
• Provide day‑to‑day service delivery and support staff understanding of client needs.

Required profile

• Certificate IV in Aged Care and at least 2 years of relevant experience.
• Knowledge of Home Care Packages, CHSP, Aged Care Quality Standards and Commonwealth Home Support Programme.
• Current driver’s licence and willingness to travel between client sites.

Required skills

• Strong administrative and computer skills.
• Ability to operate effectively in a multi‑site environment.
