Care Integration Nurse - Home Hospital

About the role

Amplar Health, part of the Medibank Group, is recruiting a Care Integration Nurse for its Home Hospital (AHHH) program. The nurse works with Medical Officers, Nurse Care Coordinators, the service provider network and Local Health Networks to deliver patient-centred care to suitable patients, including clinical reviews within a multi-disciplinary team. Visits are made to patients' homes using a company vehicle, starting each shift from the Eastwood office.

The position is casual, with rostered hours across a 7-day roster in two shifts: 7am to 3pm or 2pm to 10pm.

Key responsibilities

• Deliver patient-centred nursing care to suitable patients for Amplar Health Home Hospital, according to their care plan.
• Undertake clinical reviews as part of a multi-disciplinary team.
• Visit patients in the community at their homes and use the company vehicle for patient visits.

Required profile

• Bachelor of Nursing and current unrestricted AHPRA registration.
• Minimum 3 years' experience as a qualified Registered Nurse.
• Current Basic Life Support certification.
• Flexible availability to work a variety of shifts.
• South Australian Working with Children Check and National Police Clearance (or eligibility to obtain them).
• Willingness to provide proof of up-to-date vaccinations if requested.

Required skills

• Clinical experience managing acute medical and surgical patients, intravenous cannulation skills, or willingness to achieve this competency.
• Experience with comprehensive clinical assessments is highly regarded.
• Understanding of the National Safety and Quality Health Service Standards.
