Care Coordinator – Home Care

About the role

Healthcare Australia is looking for a people‑focused Home Care Coordinator to support older Australians in the Adelaide area. In this client‑facing position you will build strong relationships, develop personalised care plans and act as the trusted point of contact throughout the Home Care journey.

Key responsibilities

• Monitor service delivery through regular communication, care‑plan reviews and client feedback.
• Collaborate with Registered Nurses and Allied Health professionals to coordinate care for clients with complex needs.
• Work closely with internal teams (Support Worker Service Delivery Lead, Recruitment and Rostering) to ensure efficient service delivery.
• Support workforce planning by contributing to the recruitment and matching of suitable care workers.

Required profile

• Experience in Home Care, aged care, community services, healthcare or case management.
• Organised, proactive and able to manage multiple priorities while maintaining accurate documentation.
• Excellent communication skills with a customer‑first mindset.
• Ability to build strong relationships with clients, families and colleagues.
