Support Co-ordinator / Specialist (NDIS) - Flexible, Work From Home

About the role

Inclusion Tree, a national NDIS provider offering human-centred, strengths-focused and trauma-informed supports, is looking for a Support Co-ordinator / Specialist. The role is a casual position of up to 38 hours per week, combining work from home with community engagement, and offers a path to permanent employment. It sits under the Social, Community, Home Care and Disability Services Industry Award.

Key responsibilities

• Help participants coordinate and implement their own supports and take part more fully in the community.
• Adjust supports regularly as participants' needs change, and link formal, mainstream and community services.
• Facilitate multiple, complex supports across different providers and resolve crises while building long-term resilience.
• Build capacity with tools and resources that belong to the participant, and monitor outcomes and report to the participant and the NDIA.
• Use the NDIS budget creatively to implement the plan and achieve goals, and build confidence in the participant and their support network.

Required profile

• Diploma or Certificate IV in Community Services, Disability or a related field, or equivalent lived or professional experience.
• Good understanding of the NDIS and experience working with people with disability, their families and community networks.
• Commitment to strengths-based, person-centred practice.
• Ability to self-manage, prioritise and meet deadlines, with project-management style organisation.
• Completion of NDIS Quality and Safeguards Commission training, worker screening, and the relevant state Working with Children's check.

Required skills

• Intermediate to advanced Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Excellent verbal and written communication.
