Social Worker - Toowoomba

About the role

Proactive Health is recruiting a full-time Social Worker for its Toowoomba clinic, part of a national network of allied health businesses. The caseload covers DVA, NDIS, Home Care Packages, workers' compensation and private clients, working alongside physiotherapists, exercise physiologists, OTs and dietitians in a multidisciplinary team.

Key responsibilities

• Complete psychosocial assessments and develop goal-focused intervention plans
• Provide counselling and therapeutic support to individuals and families
• Help clients navigate DVA, NDIS, aged care and community services
• Advocate for clients and connect them with housing, financial, legal and community supports
• Work with the clinic team, GPs and external providers
• Write clear, timely reports for funding bodies and referrers
• Keep clinical documentation in line with AASW standards and Proactive Health's governance framework

Required profile

• Bachelor or Master of Social Work accredited by the AASW
• Eligibility for AASW membership
• Current professional indemnity insurance, or willingness to obtain it
• Current driver's licence and reliable vehicle
• Current National Police Check and NDIS Worker Screening Check, or willingness to obtain them
• Current Queensland Blue Card, or willingness to obtain it
• Nice to have: AMHSW accreditation, DVA or veteran client experience, NDIS or aged care experience, and a trauma-informed practice background
