Social Worker - At Home Subacute Team

About the role

Bayside Health is recruiting a Grade 2 Social Worker (Year 1 to Year 4) for its At Home Subacute team, which provides specialist rehabilitation, geriatric evaluation and management and palliative care in the community. The position is fixed term and part time, 48 hours per fortnight until December 2027, based in Frankston. It suits an experienced clinician who can work autonomously and deliver evidence-based care to clients, families and carers across home, clinic and telehealth settings.

Key responsibilities

• Undertake comprehensive psychosocial assessments and manage complex cases
• Develop intervention plans and contribute to multidisciplinary care planning
• Visit clients in their homes to provide assessment, counselling and support
• Work with families, carers and community services to achieve meaningful outcomes
• Balance the week with home visits, telehealth and clinical consultation

Required profile

• Membership, or eligibility for membership, with the Australian Association of Social Workers (AASW)
• Previous social work experience in a public healthcare setting, with a strong understanding of community support services
• Ability to independently deliver social work services including assessment, counselling, case management and discharge planning

Required skills

• Highly developed interpersonal, communication and relationship-building skills
• Ability to work autonomously and deliver high-quality, evidence-based care
